Kathleen R. Cook, 66, of Clearfield, and originally from AZ, died on Sunday, December 5, 2021 at her home.
She was born on June 20, 1955 in Ft. Defiance, AZ, a daughter of Elsie (Gatewood) Sandoval of AZ the late Henry Johns Sr.
Mrs. Cook enjoyed knitting, crocheting and puzzles.
In addition to her mother Elsie, she is survived by her husband Anderson Cook of AZ, three daughters, Gina Gram and husband David of Clearfield, Adrianne Buze and husband Kevin of AL, and Roxanne Cook-Severson and husband Mitchell of OK; eight grandchildren; two sisters, Julia Grove and husband Tom of OK, and Katherine Paz of AZ; and a brother, Kevin Johns Jr. and wife Virginia of NM.
She was preceded in death by her father, Henry Johns Sr.
All services will take place in Arizona at a later date.
